I spent 40 minutes but still couldn't get this code working.

For S&H use drop down menu - it will cost between $1.5 to $2 depends of the carrier.
Code works fine.
You should work harder pal.

okay is this for a single page 12-month calendar or a 12-page 12-month calendar?

also, spent near an hour...when to checkout and opps unexpected error...tried 4 times...uggggg....good night.

This is for a single page calendar only. The multi-page ones look to be $10. ;-(
I suppose it is a good deal if you like the single page style.
